Laboratory services

Most laboratory blood draws and specimen collections can be performed during your appointment. Our lab drawing and processing is exclusively for our own patients. To ensure accuracy, we are unable to process orders written by other doctors for laboratory services or blood draws. We thank you for your understanding.

After growing up in the DMV, she moved to New York to study linguistics at Bard College and had a career in cognitive neuroscience research before pivoting to medicine. While at New York Medical College, she chose to pursue family medicine to best serve her community. Her interests within the field include geriatrics, mental health, and medical ethics.

Dr. Casal-Roscum is devoted to helping seniors enjoy their older years via preventative care and excellent management of chronic conditions. She strives to support patients and their families through the aging process.

In her free time, she loves spending time outdoors, reading, traveling, and trying new restaurants.

Jason A. Cooper, MD

Jason A. Cooper, MD is a board-certified family medicine physician with nearly 30 years of experience, dedicated to providing compassionate, comprehensive care to patients of all ages in Herndon, Virginia. Since joining Herndon Family Medicine in 1998, Dr. Cooper has been recognized for his clinical excellence and patient-centered approach, earning multiple accolades, including being named a “Top Doctor” by Washingtonian and Northern Virginia magazines numerous times.

Dr. Cooper graduated magna cum laude and Phi Beta Kappa from Brandeis University in 1991. He earned his medical degree cum laude from the University of Maryland School of Medicine in 1995, where he was inducted into the Alpha Omega Alpha Honor Medical Society. He completed his residency in family medicine at Fairfax Family Practice Centers, serving as Academic Chief Resident from 1997 to 1998.

Dr. Cooper’s dedication to patient care is evident in his attentive bedside manner and commitment to spending quality time with each patient. His approach has garnered high praise from patients, who appreciate his thoughtful, individualized care.

Beyond his traditional medical practice, Dr. Cooper is passionate about integrative health. He was certified in mind-body medicine through The Center for Mind-Body Medicine in 2021 and became an Usui Reiki Master Practitioner in May 2024. In 2023, he founded Mind Body Healing Arts, LLC, to offer holistic services that promote self-care and well-being. In addition to facilitating virtual eight-week mindfulness skills group sessions, Dr. Cooper also sees individual clients in person on Fridays, teaching techniques to reduce stress and anxiety, enhance performance, and increase joy. He has also provided mindfulness coaching to Inova family medicine residents, adults, adolescents, and high school athletic teams. 

Outside the office, Dr. Cooper enjoys hiking, traveling, meditating, and spending time with his wife, his two shih tzus, and two adult sons.

Before becoming an NP, she worked in Labor and Delivery at Fairfax Hospital, where she developed a deep passion for women’s and newborn health—an area she continues to focus on in her practice.

Megan provides compassionate, evidence-based care to patients of all ages, with a special interest in women’s health and family wellness. Her approach blends clinical expertise with a warm, supportive presence, and she is committed to empowering her patients through education and partnership in care.

Outside of work, Megan enjoys spending time with her husband, family, and their German Shepherd, Stella. A loyal Washington Commanders fan, she loves relaxing Sundays filled with football and family.

Michelle Hewlett is a certified Family Nurse Practitioner committed to delivering holistic, patient-centered care to individuals and families of all ages. Known for her patient-centered approach, Michelle focuses on empowering patients through education and preventive care. She believes in fostering long-term relationships with families.

Michelle Hewlett earned her Bachelor of Science in Nursing (BSN) from George Mason University in 2019 and completed advanced training with a Master of Science in Nursing (MSN) from James Madison University in 2023, specializing in family practice. She is certified by the American Academy of Nurse Practitioners Certification Board (AANPCB).

She provides comprehensive primary care services including annual physical, well-child visits, chronic disease management (e.g. diabetes, hypertension, asthma), women’s health exams, minor procedures, and urgent care for common illnesses and injuries.

Deborah I. Leavens, MD

Deborah Leavens joined Herndon Family Medicine in 1986 as one of the three founding physicians. She is a native of Northern Virginia and graduated from the University of Virginia in 1975 and The Medical College of Virginia/Virginia Commonwealth University in 1979. She did her Family Medicine residency at Fairfax Family Practice Centers, at that time located in Vienna, Virginia. After residency, she joined the Army, serving four years in various locations in Germany. After her service, she returned to Northern Virginia and has been with Herndon Family Medicine since that time. She enjoys reading, hiking, gardening, toy poodles, and is active in her church.

Kevin J. Lee, MD

Dr. Kevin Lee is a board-certified family medicine physician who believes in the concept of the old-school family doc. He believes in getting to know his patients and their families and providing more personalized care based on a deeper collaborative relationship.

He grew up a military brat and has lived all around the world, however, he has called Fairfax, VA his home for the majority of his life.

His education is as follows:

Dr. Lee specializes in the full scope of family medicine with particular emphasis on pediatrics and taking care of the whole family. He also has a heart for taking care of our veterans, as he is the child of a retired Army Warrant Officer and served as an Armor Officer in the United States Army.

He is married with 3 young kids and is active in his church community. When he is not at work, he is usually involved with his kids and likely traveling to a lacrosse tournament or cheer competition.

Dr. Lee is a native Korean speaker and is able to speak conversational Spanish.

Born and raised in Hunan, China, Dr. Li moved to the United States at age 16 and attended McLean High School before completing both her undergraduate and medical education at the University of Virginia. She completed her residency training at Fairfax Family Medicine and has been practicing at Herndon Family Medicine since 2021.

Drawn to the continuity of care and the opportunity to build lasting relationships with patients, Dr. Li chose family medicine to make a meaningful impact across all stages of life. She emphasizes shared decision-making and practices evidence-based medicine to ensure each patient receives care that aligns with their values and the latest clinical guidelines. Her approach focuses on preventive care, patient education, and personalized treatment plans in a supportive environment. Patients often describe Dr. Li as a good listener who takes time to explain things clearly and is thoughtful and detail-oriented in her care.

Dr. Richard Uribe is a board-certified and fellowship-trained primary care sports medicine physician, specializing in the prevention, diagnosis, and non-surgical treatment of musculoskeletal injuries and conditions. With a strong background in family medicine, Dr. Uribe is dedicated to helping patients of all ages and activity levels—from elite athletes to weekend warriors—return to peak performance.

Dr. Uribe earned his medical degree from University of North Carolina at Chapel Hill and completed residency training at Inova-VCU Fairfax Family Practice. He pursued a fellowship in sports medicine at VCU-Bon Secours Mercy Health in Richmond, VA, where he gained advanced expertise in the management of acute and chronic sports-related injuries, concussion care, ultrasound-guided procedures, and injury prevention strategies.

Gwen Chu, DPT

Specialty: Physical Therapy

Gwen Chu is a licensed physical therapist and has been practicing since 2007. 

Gwen received her master’s in physical therapy from George Washington University in Washington, DC, in 2007 and shortly after received her doctorate in physical therapy from Boston University. Gwen specializes in general outpatient orthopedics and is certified in manual therapy, functional dry needling and the Graston Technique. 

Gwen believes in treating the patient as a whole by focusing on the root cause of the dysfunction, rather than just the symptoms, to yield long-lasting results. Gwen continues to pursue continuing education and advanced certifications to provide the best evidence-based practice.

Andrew Pho, DPT

Andrew Pho is a licensed physical therapist and has been practicing since 2013. 

Since joining Herndon Family Medicine in 2013, Andrew Pho continues to assist and facilitate patients to reach their maximum rehabilitation potential, through evidence-based medicine and compassionate care. He believes in integrating manual therapy with therapeutic exercise and activities.

Andrew graduated from George Mason University with a BS in Biology and received his Doctorate in Physical Therapy at New York Medical College. Andrew is certified in Fascial Movement taping and Level 1 Functional Dry Needling.
