Dr. Rachel Choi Bak is a board-certified family medicine physician with a passion for providing comprehensive, compassionate care across all stages of life. She focuses on long-term health and preventative medicine and partners with her patients to support their physical and emotional well-being.
After earning her medical degree from George Washington School of Medicine and Health Sciences, Dr. Bak completed residency training in family medicine at Fairfax Family Practice Centers. After growing up in the DMV, she moved to New York to study linguistics at Bard College and had a career in cognitive neuroscience research before pivoting to medicine. While at New York Medical College, she chose to pursue family medicine to best serve her community. Her interests within the field include geriatrics, mental health, and medical ethics.
Dr. Casal-Roscum is devoted to helping seniors enjoy their older years via preventative care and excellent management of chronic conditions. She strives to support patients and their families through the aging process.

Jason A. Cooper, MD is a board-certified family medicine physician with nearly 30 years of experience, dedicated to providing compassionate, comprehensive care to patients of all ages in Herndon, Virginia. Since joining Herndon Family Medicine in 1998, Dr. Cooper has been recognized for his clinical excellence and patient-centered approach, earning multiple accolades, including being named a “Top Doctor” by Washingtonian and Northern Virginia magazines numerous times.
Dr. Cooper graduated magna cum laude and Phi Beta Kappa from Brandeis University in 1991. He earned his medical degree cum laude from the University of Maryland School of Medicine in 1995, where he was inducted into the Alpha Omega Alpha Honor Medical Society. Before becoming an NP, she worked in Labor and Delivery at Fairfax Hospital, where she developed a deep passion for women’s and newborn health—an area she continues to focus on in her practice.
Megan provides compassionate, evidence-based care to patients of all ages, with a special interest in women’s health and family wellness. Her approach blends clinical expertise with a warm, supportive presence, and she is committed to empowering her patients through education and partnership in care.

Michelle Hewlett earned her Bachelor of Science in Nursing (BSN) from George Mason University in 2019 and completed advanced training with a Master of Science in Nursing (MSN) from James Madison University in 2023, specializing in family practice. Deborah Leavens joined Herndon Family Medicine in 1986 as one of the three founding physicians. She is a native of Northern Virginia and graduated from the University of Virginia in 1975 and The Medical College of Virginia/Virginia Commonwealth University in 1979. She did her Family Medicine residency at Fairfax Family Practice Centers, at that time located in Vienna, Virginia. After residency, she joined the Army, serving four years in various locations in Germany. After her service, she returned to Northern Virginia and has been with Herndon Family Medicine since that time. She enjoys reading, hiking, gardening, toy poodles, and is active in her church.

Dr. Kevin Lee is a board-certified family medicine physician who believes in the concept of the old-school family doc. He believes in getting to know his patients and their families and providing more personalized care based on a deeper collaborative relationship.
He grew up a military brat and has lived all around the world, however, he has called Fairfax, VA his home for the majority of his life.

Dr. Lee specializes in the full scope of family medicine with particular emphasis on pediatrics and taking care of the whole family. He also has a heart for taking care of our veterans, as he is the child of a retired Army Warrant Officer and served as an Armor Officer in the United States Army.
He is married with 3 young kids and is active in his church community. When he is not at work, he is usually involved with his kids and likely traveling to a lacrosse tournament or cheer competition.

Gwen Chu is a licensed physical therapist and has been practicing since 2007.
Gwen received her master’s in physical therapy from George Washington University in Washington, DC, in 2007 and shortly after received her doctorate in physical therapy from Boston University. Gwen specializes in general outpatient orthopedics and is certified in manual therapy, functional dry needling and the Graston Technique.

Starting October 1, due to changes in federal legislation, Medicare, Medicare Advantage, and Tricare may no longer cover telehealth services. If you are on a Medicare Advantage plan, please check with your plan or your office to confirm coverage. Our staff may contact you if your upcoming telehealth appointment needs to shift to in-person. Our practice still accepts these health plans and will continue offering telehealth, but some services may not be paid for by your insurance. Congress is still considering telehealth coverage, and we will update you as we learn more.
