Before your first appointment, you’ll be asked to complete a Lifestyle Medicine Assessment Form. Your initial visit will then take about 40 minutes, during which you’ll receive: 

  • A personalized lifestyle assessment tailored to your specific goals 
  • Review of your labs, health history, and goals health priorities 
  • Education on the 6 pillars of well-being, focusing on the ones most relevant to your health goals  
  • Achievable action steps 

Depending on the cadence you and your provider agree on, you will schedule visits every 1-6 weeks to track progress, address challenges, adjust goals and coordinate care with your primary care provider.
