Keri Bortz is an adult and gerontology primary care nurse practitioner. She joined Family Medicine of Clifton and Centreville in 2021. Keri brings experience in primary care, long-term care, chronic disease management, and hospice and palliative care in the home, facility, and hospital settings. She incorporates a desire to help people through health promotion and chronic disease management. Keri enjoys working with the geriatric population and possesses great interpersonal communication skills to build positive patient-and-provider relationships and provide primary healthcare in clinic and patient homes.
