Hannah Adams, FNP-C

About
Hannah is a certified family nurse practitioner. She comes to FMCC with 7 years of experience as a cardiac critical care nurse. She is passionate about preventative care and helping patients of all ages stay out of the hospital so they can be with their families and loved ones. Hannah grew up in Northern Virginia and went to the University of Virginia for nursing school (go Hoos!), where she met her husband. They welcomed a baby boy to their family in the summer of 2025. Outside of work, Hannah enjoys walking outdoors with her husband and son, reading, volunteering at church, finding the best local ice cream shops, and sipping on a cold Coke Zero! She is excited to start her career as a nurse practitioner at FMCC.

Record Requests

Patients can securely request medical records via our release-of-information partner, Datavant. Please note that state and federal laws allow for additional fees for copies of medical records that are not requested by healthcare providers for direct patient care. To reduce costs, patients should consider requesting specific information rather than a complete record.
